Corporate Individual Application

Each individual applying for membership at the corporate rate should download the application below, complete it and submit it to the appropriate corporate office for consolidation with other individual applications. The consolidated applications (three or more) should then be mailed along with a Corporate Rate Application and payment to AAMAS at the address on the right.
